Health and Wellbeing is committed to creating positive change at UIC.
This means ensuring our services and resources meet our current student needs. This also means advocating for campus level changes that are needed to support our students’ overall health and wellbeing. But to do this, we need YOUR help.
Public Health Initiatives has developed the Spark Check Annual Health and Wellbeing Survey to ensure leadership has an ongoing and comprehensive understanding of our campus’ health needs. To ensure we capture the diverse health and wellbeing needs of our student population, we need as many students to participate in this survey as possible!
By sharing your experiences, YOU can contribute to improving the overall health and wellbeing of all our Flames, making campus a healthier and more supportive environment for everyone.
Spark Check 2025
2025 Spark Check Survey: Monday, April 7th to Friday, May 2nd
The Spark Check Health and Wellbeing Survey assesses student physical health needs and health behaviors. The survey will also strengthen what we know about how our students access and use services to support their health and wellbeing, both on and off campus.
The Spark Check Survey will be used to tailor our existing services, advocate for new services, update leadership on changes in college health that impact academic success, and advocate for changes to policies and practices so that the University of Illinois Chicago can be more supportive and health affirming for its students.

Survey Instructions
Student Eligibility
All undergraduate, graduate, and professional students* at UIC (including our satellite campuses) are invited to participate in the 2025 Spark Check Survey.
Student well-being is a collective effort. So, we want as many Flames to participate as possible. Understanding our campus’ diverse health needs can lead to positive changes to the student experience at UIC.
*Students must be at least 18 years old to participate.
Participation
All undergraduate, graduate, and professional students* at UIC (including our satellite campuses) are invited to participate in the 2025 Spark Check Survey.

Public Health Importance
Your voice truly matters.
The Spark Check Survey allows us to identify key areas where our students need support. It also allows us to modify or create targeted resources that are reflective of our students’ needs and are more effective for our students. Improving health programs and resources at UIC can improve your health or the health of your peers. In other words, your participation in these surveys plays a key role in helping us understand student life and strengthen our community.
Students who complete the survey will also be entered into a drawing for one of over 2,000 $20 Amazon gift cards or one of fifty $100 Amazon gift cards.
Survey Overview
The survey will ask questions about:
• Demographics (i.e., age, gender, type of health coverage).
• Physical health (i.e., your self-assessed health, stress, chronic conditions)
• Health behaviors (i.e., physical activity, sleep, vaccination)
• How you access and use health services both on and off campus

Survey Relevance
We know that UIC students are often asked to complete numerous surveys each academic year. Each survey aims to understand the strengths and challenges of the UIC student experience so that we can improve our policies, services, campus experience, campus life, and campus culture.
The Spark Check Survey has a unique focus on student health and wellbeing. The 2025 Spark Check Survey will fill a gap in data related to our student’s physical health needs and behaviors. This information is not captured in other university assessments.
